Drug Testing Consent Form

This gives consent for a student to participate in random drug testing.

MESH Physical Form

This gives basic information Mobile Expanded Mobile Health unit so that a physical may be provided.

Sports Physical Form

The is a form to give to a doctor, or MESH, to determine if a student is eligible to participate in WWHS athletics.

NCHSAA Injury Return to Play Form

This form must be filled out by a licensed medical professional after an injury.

NCHSAA Illness Return to Play Form

This form MUST be completed if the student is out of practice for at least five days.

Prescribed Appliance Form

This form must be completed by a licensed medical professional is an athlete is required to wear any special equipment for medical reasons.

Skin Lesion Form (Wrestling)

This form MUST be completed when there is a noticeable skin lesion.
