NCHSAA Physical Form(MUST be filled out completely or will be returned to be filled out in
its entirety)
This is the form to give to a doctor, or MESH, to determine if a student is eligible to participate in WWHS athletics.
Inhaler and Epi-Pen Policy
For sports participation, we ask that the athlete provides an inhaler and/or their Epi-Pen to the ATC for storage in case of an emergency so we know exactly where it may be.
