Concussion Forms

This form is required to be completed by a physician to be fully cleared to play.
 
To be completed after symptom free of concussion symptoms.
 

Red Flags

If any of these red flags are seen, it is highly recommended that the student-athlete is to be taken
to the emergency room immediately. 
  • Loss of Consciousness
  • Seizures
  • Excessive Vomiting
  • Severe Neck Pain
  • Numbness or Tingling in Extremities
 

Signs and Symptoms

(anything more than NORMAL)
  • Headache
  • Pressure in Head
  • Nausea and/or Vomiting
  • Dizziness
  • Blurred Vision
  • Balance Problems
  • Sensitivity to Light and/or Noise
  • Feeling Slowed Down and/or Like “In a Fog”
  • Difficulty Concentrating and/or remembering
  • Fatigue/Low Energy
  • Confusion
  • Drowsiness
  • More Emotional
  • Irritability
  • Sadness
  • Nervous/Anxious
  • Trouble Falling Asleep
  • Etc.
